Please use the following online forms as a way of providing information to CTec.

• Concern Form- Use this form if you would like to file a technology-design or functionality concern.
• Problem Report Form- Use this form if you would like to file a classroom problem report. (If emergent, PLEASE call 293-CTEC extension 0 for immediate attention)
• Student Employment Form- Use this form if you are a current WVU undergraduate looking to apply for a position.

Please use the link and information below to provide feedback for the faculty survey on classroom renovations and technology.  Your comments and your assistance in encouraging other instructors (including GTAs) to participate are greatly appreciated.  Feel free to circulate the survey through email to colleagues, departmental listservs, or other means.

• Faculty Survey on Classroom Renovations and Technology - WVU has invested significant resources in classroom renovations and technologies. These renovations are intended to improve the teaching and learning environment by enabling instructors to integrate technology seamlessly and by providing visual, audio, and physical resources to facilitate student learning. We are seeking your feedback regarding centrally-scheduled rooms in need of renovation, the technology resources desired in classrooms, and barriers to using technology. This survey should take less than ten minute to complete, and all responses are anonymous. Please complete the online survey at http://simpleforms.scripts.wvu.edu/2010plan/ctec/ .
